how do u go about getting ur elderly parents into a nursing home

there both 79 yrs old, need full time supervision. do i go through there doctor, do i go through a social worker?? plz help we live in nyc
these r my boyfriends parents, he has a sister that is married but she and her husband dont do anything for them, so it is all on him & its hard for him to work & take care of them.so hes looking for other options
