What's a good way to thank nursing home staff?

My Grandmother is being discharged from her nursing home in about a week and is moving back to Assited Living. This has been her third stay in this particular nursing home, and each time she's been there, the care has been wonderful. The nurses and CNA's are all warm, kind people and take excellent care of her. We are so greatful for everything they've done. There are a few CNA's and nurses that have really gone above and beyond for her, and we want to do something for them. Any ideas?
