I have a CNA (Certified Nursing Assistant) certificate. NYS. How to renew?

I have a CNA (Certified Nursing Assistant) certificate. New York City.
It expires in April. I don't work as a CNA right now.
Where should I go to renew it? To the department of health? Does anybody know the exact address in New York where should I go to renew it?
